The following documents are required to be on file in the school office before your child may attend Arlington Country Day School:

Original Certified Birth Certificate

Florida Certification of Immunization or Exemption Florida Department of Health
NOTE: Incoming records of students transferring to our school from schools out of the county or state will be checked to see if the immunization records are in compliance with Florida State regulations. The parents of students with immunization records not in compliance will be notified. Failure to meet the regulations will result in the student being excluded from school until the students immunizations are up to date.

Florida Certification of School Entry Health Exam (Completed within one year of the date of enrollment. Forms are available at physicians' offices and the Duval County Public Health Unit)
